Director, Client Advocacy
 

Position Description: The Director, Client Advocacy, is responsible for all grant deliverables and program quality assurance for client advocacy services at GMHC. The Director is responsible for ensuring timely and effective client advocacy services in the areas of insurance, health care access, and benefits. The Director, Client Advocacy will oversee several programs including but not limited to the AIDS Institute Funded ADAP Minority AIDS Initiative Program and the NYSDOH Health Navigator Program, as well as funding from private funders and foundations.

Group Facilitator (TGNCNB Program)
 

Position Description: The Group Facilitator will assist with the coordination and implementation of all group level interventions for this new multi-year grant dedicated for Transgender, Gender Non-Conforming and Non-Binary community members. The individual will be responsible for the day-to-day coordination, recruitment, and engagement of clients for these interventions. The Group Facilitator will plan and provide direct facilitation as well as support clients with linkage and navigation services.

Healthy Aging Specialist
 

Position Description: The Healthy Aging Specialist will serve the Aging, HIV positive LGBTQ client population by providing a range of services to improve the overall health and wellbeing of that population. Such services will include individual mental health and substance use screenings, brief intervention, and referrals. The Healthy Aging Specialist will provide educational material and workshops relevant to the health of Aging LGBTQ clients with HIV including physical, dental and mental health. The Healthy Aging Specialist will provide Buddy services to high-need clients.

HIV Test Counselor (TGNCNB Program)
 

Position Description: The HIV Test Counselor/Phlebotomist will assist in the coordination and implementation of counseling, testing and referrals for participants in the off-site and on-site settings. Assist in testing at outreach efforts to the priority populations, including counseling, testing and referral services and managing CTR administrative responsibilities. This position will also assist with targeted off-site outreaches specifically tailored for Transgender, Gender Non-Conforming and Non-Binary community members.

Position Description: The Terry Brenneis Hub for Long-Term Survivors (LTS) is a program to support people who identify as long-term HIV survivors in their efforts to lead healthy and rewarding lives. The LTS Hub also provides support under a program shared with ACRIA Centers at GMHC called the AIDS Survivor Syndrome Program (ASSP).

Monitoring & Evaluation Specialists
 

Position Description: The Evaluator provides capacity and support for supporting agency-wide data-driven management processes, research activities, evaluation and quality improvement initiatives. The Evaluator will support programs in identifying key quality and management indicators and creating monitoring processes. This individual will evaluate these programmatic efforts, collecting and analyzing agency data in order to produce research that will contribute to the knowledge base of effective program management, quality improvement, and HIV care. The Evaluator will also work with programs to plan, support, and monitor their usage of the Electronic Health Record.

Navigation Specialist, The Wellness Center (Part-Time)
 

Position Description: The Navigation Specialist's primary role is to connect clients to services within GMHC and the greater community. The Navigation Specialist will assist with client check in and the monitoring of the NMW reception area and dining room during meals service, providing oversight and client support. The Navigation Specialist will link clients in NMW to services in other GMHC departments as needed. In addition, the Navigation Specialist will provide clients with linkages to external services as needed. The Navigation Specialist will work with the NMW program staff in collecting contractually required documentation and inputting information, such as primary care status measures and client related updates, into GMHC's client databases as required.

Position Description: The Peer Navigation Ambassador is responsible for conducting outreach, escorting clients to appointments, conducting linkage navigation services, and conducting re-engagement efforts.

Peer Navigation Specialist (TGNCNB Program)
 

Position Description: The Peer Navigation Specialist will assist with the implementation of group level interventions for this new multi-year grant to dedicated for Transgender, Gender Non-Conforming and Non-Binary community members. The Peer Navigation Specialist will act as a role model, sharing personal strengths and skills, while promoting hope and recovery. The Peer Navigation Specialist must be knowledgeable of community resources available to clients internally and externally. Duties may also include administering surveys, data gathering, maintaining participant databases, assisting with event organizing, and performing various administrative tasks.

Social Work Case Manager
 

Position Description: The Social Work Case Manager is responsible for providing case-specific and clinical supervision. Staff Attorney
 

Position Description: The Staff Attorney provides civil legal services to GMHC clients in a range of legal areas, including landlord-tenant law, family law, HIV confidentiality, bankruptcy, advance directives, employment law, food stamps, social security and other entitlements, HIV discrimination law, disability law, and health care access. Tracks all activities, including, the provision of services and the achievement of outcomes in appropriate daily logs, an electronic database, and an individual dashboard. Assists with the preparation of monthly and other reports, and works to make sure that the department is audit-ready.

Transgender Nonconforming Specialist
 

Position Description: The Transgender Nonconforming Specialist has the overall day-to-day responsibility of the delivery of care coordination services. These services include comprehensive case management, care coordination and health promotion, comprehensive transitional care from inpatient to other settings, individual and family support, referral to community and social support services, facilitate gender-affirming hormone therapy through vouchers and health insurance navigation, and the use of health information technologies.
